I don't doubt that you could excise, rotate and paste in the teat in Photoshop, but what I do doubt is that it would occur to you or to anyone else to do so when it would be so much easier to leave the teat straight and just paste the lamb so that it lines up with a straight teat. As for the shadows and illumination, the sky is overcast and the light is soft. Shadows are diffuse and the lamb is a whiter subject than the cow. I'm still not convinced.

I say it's photoshopped.
