I remember when they were working on the Mouth of Sauron sequence (for the RotK:EE), Weta tried all these different ways of doing the actual mouth. One of the DVD extras shows some of the ways they tried, including turning it upside down with CGI, and even sideways (which looked hella weird, but not in the creepy way they were going for).
They finally just made it bigger, so it winds up taking up like half of the actor's face.
