If it is, I'd guess it's just the seal that's shopped on.
If more than that's a photoshop, my compliments to the creator; the highlights and shadows on the underwear match the other light sources and the focus incredibly well.
My guess would be that it's a Speedo swimtrunk or a bikini bottom, and the seal was 'Shopped onto it. He's really holding it though, unless his hands are 'Shopped in, too.
Note that it's translucent: you can see his fingers through it, although they appear to be a bit longer than they should, but that may be from his pushing them forward to stretch the material.
It's not a Photoshop. I read the story attached to this a week or two ago. The garment in question is swimtrunks, thought I am not sure I remember the Great Seal on the front or not. I also seem to remember him being in Britain or some other Third World country at the time.
But, yes, Bush is holding up what looks like shiny, black underwear fo' reals.
