If you look closely, you can see the pores of the skin and some slight scarring through the ink. The scarring particularly is indicative of it being a real tatoo - and an f-ing cool one, though not my choice of subject matter...

I've seen definitely real tattoos like that before, on hands (AGH GET IT OFF GET IT OFF AGH) and feet, so I know they're possible. I've never seen one this big, but I believe it.
