Don't know, it looks to me like someone shaved off the layers of skin and fat right down to the muscle and then just covered it with a thin layer of skin.
My half-educated guess is QuikClot. (Best way to stop diffuse bleeding from large soft tissue damage in a war zone, but it generates heat and leaves disfiguring scars.)
Part of my brain goes "oh dear god gross" while the other part knows that if I ever got such a scar, this kind of tattoo would be the first thing I went out and got.
