Um... Hospitals in the US rent out their OR's to doctors who compete for patients entering Emergency. There are several beds in each OR as surgeons operate on as many patients as possible simultaneously. Heart monitors and respirators are old and dilapidated, scalpels are rarely sharpened and clamps and needles are just rinsed quickly in diluted iodine before getting reused. On the other hand, the anesthetic gas vaporizer is kept in top condition as it's the only thing that keeps patients from squirming and slowing down the procedure. The OR costs $50,000 a day to rent and operations cost an average of $3,000 each so surgeons have to churn out 17 surgeries before they can consider buying a new golf club. Many surgeons poach a few tissues and spare organs from the patients to supplement their income. There is a high mortality rate among the patients, usually from septic shock. The OR's are all equipped with back doors as every so often enraged relatives of the deceased arrive at the hospital equipped with baseball bats. If the surgeon hears them coming down the hall he may escape out the back and thus avoid getting clubbed, in which case the mob trashes the OR instead. The cartels owning the hospitals get off scott free. Needless to say, they are quite satisfied with the status quo and resist all movement by the government to socialize medicine.
It's the best bus system in the world. Rich get richer, free market competition incentives for the drivers, speedy times between stops, an acceptable casualty level, and no rationing of seats.
It's the American Dream in full glaring neon lights! I bet they've dispensed with those fascist socialist traffic lights that slow my car down too. Glorious!
