It's disturbingly easy to do... a lot of antiseptics and anaesthetics are quite inflammable, you've got gauze and surgical drapes all over the place, and those surgical lamps get really hot.

If you say "I'm sorry for the pain and distress we caused you," you are admitting fault. If you say "I am sorry for any pain and distress you may have felt," you are empathizing, but not attributing blame. An malpractice inquiry will doubtless be made, but until the hospital gets the results of that, all you're going to get is "gosh, that's too bad."
The reason being, if someone says "I'm sorry we did that to you," and the inquiry turns out that it was a total freak accident that no one could have foresaw and all reasonable steps were taken to avoid it (ergo, not malpractice), the patient could still sue because the medical team accepted responsibility.
