It's disturbingly easy to do... a lot of antiseptics and anaesthetics are quite inflammable, you've got gauze and surgical drapes all over the place, and those surgical lamps get really hot.

-- Steve's glad the incidence is so low, but empathises greatly with the patient and surgical staff affected by this case.

Yes, but my guess is that the number one cause of OR/patient-related fires is oxygen.

A surgeon I know says: "alcohol cleaning solution plus electrical cauterising tools plus impatient surgeon."

"Any pain and distress"? Why not just admit the patient got hurt?

It's a matter of legal culpability.

If you say "I'm sorry for the pain and distress we caused you," you are admitting fault. If you say "I am sorry for any pain and distress you may have felt," you are empathizing, but not attributing blame. An malpractice inquiry will doubtless be made, but until the hospital gets the results of that, all you're going to get is "gosh, that's too bad."

The reason being, if someone says "I'm sorry we did that to you," and the inquiry turns out that it was a total freak accident that no one could have foresaw and all reasonable steps were taken to avoid it (ergo, not malpractice), the patient could still sue because the medical team accepted responsibility.

I was going to say, you can tell this didn't happen in the US because the hospital apologized at all.

In America, they would send you a bill with a surcharge for extra heating during winter.

Flammability is a pre-existing condition. I'm afraid it voids your coverage.
